Please bring your insurance card and the prescription written by your doctor (if you have one). If you do not have insurance, a prescription from a doctor is helpful but not required.
Please arrive 10-15 minutes early to fill out this paperwork in our office. It is required to complete all paperwork before or at your first visit.
Please dress appropriately. If you are being seen for a knee injury please wear or bring shorts; if you are being seen for a neck or shoulder injury, please wear a tank top.
