Before Your First Appointment |

We strive to make your physical therapy experience a positive one. Our insurance
specialist will work with you to ensure that your insurance claim is filed properly and accurately. There are a few things you can do in anticipation of your first appointment that will help your visit go smoothly. Please bring your insurance card and the prescription written by your doctor (if you have one). If you do not have insurance, a prescription from a doctor is helpful but not required. |

Arrive 10-15 minutes early to fill out this paperwork in our office. Please complete all paperwork before or at your first visit. Dress appropriately. If you are being seen for a knee injury please wear or bring shorts; if you are being seen for a neck or shoulder injury, please wear a tank top. If you have questions,please call. |
