At Proformance Physical Therapy, we make every effort to see that our services are covered by your insurance. We will call your insurance company the day of your first appointment to ensure that physical therapy services are covered and to determine your specific benefits. We will inform you of your benefits and your financial responsibilities.

If a co-pay is required, this must be paid at the time of service. Any remaining balance, such as a deductible or co-insurance, will be processed after payment from your insurance. We will send you a statement at the end of your 30 day billing cycle, and payment is due within 30 days.

If pre-authorization is required, we make every effort to obtain authorization prior to your first visit.

Proformance Physical Therapy is pleased to participate with most major insurance companies and networks. Contact our office to ask about your company and coverage.

Knowledge of your personal health insurance coverage is your responsibility. If you have questions about insurance coverage, we recommend that you contact your insurance company prior to your first appointment.

As a courtesy to our patients, we will submit claims to your primary and secondary insurance carriers.
